ENT Disorders - Ear Nose and Throat Problems and Treatments


Because these three organs are inter-connected, picking up a problem in one of the three can cause a host of different problems in the other two; this is where the ENT specialist steps in as he is well versed in the treatments of any ENT disorders and the related illnesses that these problems can cause.

Symptoms


There are far too many disorders to list; this article is focused on a few of the common ENT ailments that most of us have had before:

1) Sinusitis - probably the most common, this is the congestion of the nasal passages, accompanied by headaches, sneezing, postnasal drip and a runny nose. Quite unpleasant but easy enough to solve [if there is no underlying serious medical condition].

2) Swimmers ear - if the earlobe is tugged the patient experiences extreme pain and the patient may have pain while chewing accompanied by swollen lymph nodes. Can be solved with over the counter medication.

3) Inner ear infection - there may be a feeling of increased pressure in the ear with a fair amount of pain, this is probably caused by a fungal infection in the inner canal and will need immediate attention.

4) Ear infection - dull pain accompanied by loss of balance, hearing and a fluid discharge are all signs of a problem can be solved by over the counter medication but if no improvement is noted after a day or two, the patient will need to see a doctor.

5) Strep throat - fever, fatigue, difficulty in swallowing, sore throat and enlarged lymph nodes, patient needs medication for this problem and will need to see a doctor for an anti-biotic and a throat analgesic.

6) Tonsillitis - really sore throat, fever, chills, and difficulty in swallowing, this condition definitely needs medical care as it is recurring and may need surgery.

There are other throat, ear and nose infections out there but the ones above are generally the most diagnosed.

* environmental - generally due to dampness [swimmers ear] an infection could occur if no care is taken in keeping the ear canal dry and clean

* viral - affect more than one part of the body and cannot be cured with a anti-biotic

* bacterial - these infections are generally localized [affect only one side of the throat] and occur when the area is bruised or damaged in some way

* fungal - the monster of the bunch, immediate attention is needed as any delay could compound the problem due to the exponential growth of fungi

Home remedies


1) Garlic oil - has antimicrobial properties and can be used to treat an ear infection2) Ginger tea - has the same properties as garlic and can be used to soothe a sore throat3) Eucalyptus oil - used in conjunction with steam can relieve congestion and soothe inflammation in throat or nasal infections4) Gargle - salt is a mild antiseptic and can be gargled with water to treat a throat infection

The following is based on traditional Chinese medicine (TCM). TCM is based on the observation that everything is a combination of opposites: yin (cold, cleansing) and yang (hot, building), many of which we control through diet, exercise, sex, environment, thought, etc.

There are two types of ear infections, inflammation (otitis): external and internal. External otitis, also know as swimmers ear, affects the outer ear. It is generally caused by an upper respiratory infection. Otitis media affects the middle ear (behind the eardrum) and is common in infants and children. The Eustachian (connects naso-pharynx and middle ear) and auditory tubes (connect the ear to the back of the nasal cavity) regulate air pressure, temperature and moisture. Low pressure, colder temperatures tend to increase moisture, water in the ears, especially in young children, babies. Excess moisture, water in the ear canals and Eustachian tubes can accumulate, stagnate and fester, attracting and feeding bacteria and viruses that inflame and pressurize the ear causing an earache: sharp, dull or throbbing pain, feeling of fullness in the ear and or a high fever (as high as 103 F). High altitude and or colder temperatures increase discomfort and infection.

The ears (connected to the nasal cavity) are susceptible to an external or internal attack of cold and damp. Cold and damp air travels with the wind and easily penetrates the nose, mouth and ears. In the extreme, this tends to cause condensation in the ears. Cold condenses. In nature, winter cold cools, hardens and condenses water in air into rain, snow and ice. In the body, cold condenses fluids in the lungs, nose, throat, sinuses, mouth, ears, etc. into water, mucous and phlegm. Bacteria and viruses thrive in stagnant, watery mediums (mucous, phlegm, cysts, urine, etc.) before inflaming and infecting.

Middle ear infections are common in children. Children tend to be weak, cold as they are still developing. This weakness, lack of energy, heat makes them more susceptible to cold and damp, external and internal.

Internally, the body is heated in many ways via digestion, circulation, locomotion, etc. all of which are heated fueled by blood, nutrients, protein and fat. Protein and fat build and fuel all structure and function.

Digestion is a major source of heat within the body. Three meals per day activate digestion, acid, enzymes and bile in the stomach and small intestines (twenty-two feet in length), which in turn, generates heat, much in the same way, a car engine heats the car. Heat from digestion naturally rises into the lungs, throat, mouth, nose, sinuses and ears, heating and drying.

The lungs (includes throat, nose, sinuses, etc.) are naturally moist. Moisture, water facilitates the exchange of gases (oxygen, carbon dioxide. Too much or too little weakens the exchange, which in turn, disrupts the breath and or causes infection, inflammation, etc.

Weak digestion (common in children) generates less heat, causing a temperature drop and subsequent cooling and moistening (water, mucous and phlegm) of the lungs, nose, throat, etc. In nature, te cold temperatures of evening, night time thicken and harden water in the air into the morning dew. In winter, cold temperatures thicken, harden water in the air into rain, snow and ice. In the body, colder temperatures thicken, harden water in the lungs, nose, throat ad sinuses into mucous and phlegm.

Low protein, low fat (milk, yogurt, cottage cheese, beans, nuts and seeds) and high carbohydrate diets, in the extreme, weaken, cool and dilute digestion (acid, enzymes), elimination (loose stools), respiration (mucous, phlegm), ears (water, infection, inflammation), immunity (tendency to catch colds), etc. Milk and cereal are cold damp as is orange juice. Hot cereals with a little cinnamon or ginger are more warming, building.

The middle diet, meal plan including spices (cumin, coriander fennel, cayenne, ginger, etc.) is recommended. Use spices in soups, stews, desserts, etc. Spices increase digestion and dry dampness: excess fluids: mucous, phlegm, loose stools, edema, cellulite, etc. Ginger or cinnamon can be used in cookies, desserts. Vegetables (carrots, yams) and fruits (apples, pineapple, etc.) can be used as sweeteners, to reduce sugar cravings. Cooked foods, soups, stews, protein and fat stimulate, warm the body: lungs, digestive organs etc Milk, yogurt, cottage cheese, ice cream, salads, tropical fruits, smoothies, shakes and cold drinks cool and moisten the body.

Garlic or peppermint oil drops in the ear are commonly used as they dry dampness while fighting infection. Ear cones, candles are used for the same reason. The candles are placed in the ear and set on fire. As the cone, candle burns down, it absorbs water, dries the ear. There are different types of ear candles. Ear candles with wax may drip excess wax into the ears.

Inner ear dizziness affects people of all ages, male or female and ethnic background. It is among the most common problems that patients see doctors for. When disrupted the receptors will send incorrect signals to the brain saying we are moving. This results in the sensation of moving when we are still, which is dizziness and vertigo.


The feelings of inner ear dizziness is a feeling of movement when you are still. You may feel the room spinning around you. The triggering factors of BPPV are very classic. The vertigo episodes are brought on by certain head movements. Typically looking upwards, rolling over in bed, bending over and turning the head could being on an episode of vertigo. The episode of vertigo is short in duration being less than 10 seconds. It will not last for hours. There can be a feeling of nausea but actual vomiting is less common.

Usually medication is recommended to treat the symptoms. They can help lessen the symptoms because they dampen the activity of the nervous system which results in less feeling of dizziness and less nausea. But they do nothing for the particles in the inner ear that are causing the problem. So the cause of the problem is left untreated and just addressing the symptoms is the focus of medication. It is a much better, longer term solution to address the cause of the problem and the symptoms will naturally be taken care of.

There is good news for inner ear dizziness sufferers. There is a safe and effective solution that does not involve the use of drugs or surgery. This is a medically researched procedure and has been around for more than 10 years. Why it is not more commonly recommended than medication is not really known.

You could begin to notice some loss of hearing and this will be because of a blockage in the ear. You could also experience tinnitus [ringing in the ears] plus a feeling of fullness and pressure.

If the hearing loss is in a child you will probably notice that they will need the TV turned up or they might not answer or come to you when you call. They could appear as though they are not paying attention at school, but all of this could be because they are not hearing so well.

Mostly ear infections seem to come after flu or a bad cold and can possibly be accompanied by a fever.

Middle ear infections seem to mostly hit small children under six years of age. Usually after that age they will grow out of having these infections and should be alright as they are growing up, having no lasting problems.

The reason for children being so susceptible is because their tubes from ear to nose are naturally quite small and easily get blocked.

The best thing to do when children exhibit symptoms of inner ear infection is to watch and wait, the situation can clear up without any help from you, but if this is not the case then naturally you need to consult your doctor.

The pain from this affliction can be quite sharp but will not necessarily be accompanied by fever.

Mostly this inflammation and infection occurs just behind the eardrum. The Eustachian tube becomes blocked and is not ventilated properly meaning that fluids cannot drain through and out through the nose.

Ears normally produce a small amount of fluid, this is quite natural and you shouldn't even notice when it drains down your throat.

When you get such a blockage it could be due to a respiratory infection, blocked sinuses or even adenoidal problems.

If you have had a cold, the Eustachian tube, which drains any fluid from the ears into the throat, becomes swollen and blocked with mucus/catarrh. Because the fluids are not draining away properly they can become quite nasty and germs can build up as a result of this.

These inner ear infections can last for quite a few weeks before they really clear up, if they continue beyond this then you obviously need to see your doctor.

Tip #1: If you already use Q-Tips, then be sure to hold the Q-Tip 1 inch away from the cotton tip that you are inserting into your ear. This will ensure that you never make accidental contact with your ear drum, which can cause injury. The wax glands that produce the earwax are near the outer edge of the canal, so this depth should be sufficient and safe. Use a rotating motion, swabbing all sides of the canal. DO NOT use a plungering motion, which ultimately pushes and compacts the wax deeper into your canal. (FYI: the cleanest ears we see use Q-Tips daily, but only when done safely & properly.)

Tip #2: If you own a hand-held showerhead with a rotating spray pattern dial, set the dial to the most direct/condensed spray pattern. While taking a warm shower, remove the handheld portion of your showerhead and irrigate each ear for 1-2 minutes, 1-2 days per week. The warm water will liquefy the wax and expel any other unwanted debris from your ears. Handheld shower heads are available at most retail or home improvement stores.

Tip #3: Use hydrogen peroxide or pure virgin olive oil to first loosen large amounts of old wax from your canal. Allow the liquid to stand in your ear for 5-10 minutes with your head tipped to the side, and then flush with warm water from a bulb syringe or using the method shown in Tip #2 above.

Labyrinthitis is an inflammatory disorder of the inner ear or labyrinth. Common symptoms include giddiness, hearing loss, a buzzing sound in the ear, discharge from the ear and nausea or vomiting. Bacteria or viruses can cause acute inflammation of the labyrinth in conjunction with either local infection or a generalized infection.

The Ayurvedic treatment of labyrinthitis is aimed at treating the basic pathology of the disease, controlling the symptoms and preventing the recurrence of this condition. In order to treat symptoms like giddiness and nausea or vomiting, medicines like Laxmi-Vilas-Ras, Shrung-Bhasma, Laghu-Sutshekhar-Ras, Vishwa (Zinziber officinalis), Kapurkachari (Hedychium spicatium), Lavang (Caryophyllus aromaticus), Dalchini (Cinnamomum zeylanicum), Pippali (Piper longum), Haridra (Curcuma longa) and Arjun (Terminalia arjuna) are used. In order to treat the after-effects of bacterial or viral infections, medicines like Triphala-Guggulu, Arogya-Vardhini, Sukshma-Triphala, Gandhak-Rasayan and Mahamanjishthadi-Qadha are used.

In order to treat inflammation in the labyrinth, medicines like Panch-Tikta-Ghrut-Guggulu, Saarivasav, Chandanasav, Chandrakala-Ras, Saarivadi-Vati, Punarnavadi-Guggulu, Gokshuradi-Guggulu and Punarnavadi-Qadha are used. Herbal medicines which can be used in this condition are: Punarnava (Boerhaavia diffusa), Amalaki (Emblica officinalis), Haritaki (Terminalia chebula), Gokshur (Tribulus terrestris), Yashtimadhuk (Glycerrhiza glabra), Saariva (Hemidesmus indicus), Haridra (Curcuma longa), Daruharidra (Berberis aristata), Apamarga (Achyranthus aspera), Sahinjan (Moringa pterygosperma) and Guduchi (Tinospora cordifolia). Medicines like Kaishor-Guggulu, Tapyadi-Loh, Ekang-Veer-Ras, Maha-Vat-Vidhwans-Ras and Vat-Gajankush-Ras are used in order to prevent or reduce hearing loss.

With adequate treatment, most patients of labyrinthitis recover within a few days to a few weeks. It is important for such patients to refrain from driving or operating heavy machinery till they recover completely from this condition. Labyrinthitis may also be caused by potentially life-threatening conditions such as meningitis, cerebro- vascular ischemia or brain stem tumor. These conditions should therefore be carefully ruled out while treating labyrinthitis.

Endolymphatic hydrops (EH. is a baffling condition to patients and physicians alike. It is sometimes referred to as Meniere's disease, though this terminology is misleading. Meniere's disease represents a triad of symptoms described in the 19th century by French physician Prosper Meniere. Endolymphatic hydrops seems a far more helpful term to help understand the underlying problem.

Individuals with tinnitus sometimes experience intermittent feelings of fullness in one or both ears. This typically occurs in one ear at a time. The sensation of fullness may be accompanied by a fluctuating hearing loss and increased tinnitus. An audiogram shows the hearing loss to be sensorineural--involving the inner ear or auditory nerve.

The inner ear is a closed system that consists of the snail-shaped cochlea and semicircular canals, or vestibular apparatus. The entire system is encased in the temporal bone of the skull. The cochlea is concerned with hearing. It contains delicate hair cells which receive sound vibrations and transmit resulting electrical impulses via the auditory nerve to the brain. The vestibular system is concerned with balance and equilibrium. Each system is filled inside with a fluid called endolymph, in which the hair cells and vestibular structures are suspended.

If a build up of excessive fluid (endolymph) occurs, it results in an increased pressure within the enclosed inner ear structures--known as endolymphatic hydrops. The fluid-transfer mechanism in this system works very gradually, and no safety valve exists, so a significant pressure and fluid buildup may require several hours or days before it returns to normal. This increased pressure may result in a loss of hearing, tinnitus, and a severe disturbance of the equilibrium, called vertigo. Any one or all three symptoms may occur, depending upon how high the pressure becomes. Vertigo is the most disabling of the symptoms, a feeling like seasickness, where the environment is violently spinning around you, or you are spinning within your environment. Recurring vertigo from EH is often accompanied by severe nausea and vomiting, and can persist for days at a time.


Physicians have speculated for years as to the cause of the fluid buildup. Surgical procedures have been devised to relieve the pressure, and medications prescribed primarily to help relieve the vertigo, nausea, and vomiting. None of them have proven very effective.

Recent studies have implicated sensitivity to specific substances as a frequent cause for the fluid buildup. In my personal experience, wheat sensitivity is the most common culprit, followed by corn, cow-dairy products, and soy products. Others are aspartame and monosodium glutamate (MSG.) A substance sensitivity is not quite the same as an allergy to that substance, but similar. If you're truly allergic to something, the smallest amount of it may cause an allergic reaction, such as hives, wheezing, etc. Allergy is mediated through a body chemical called histamine, so medications that reduce or slow the production of histamine effectively relieve allergic symptoms. Antihistamines are not helpful for substance sensitivities, since the underlying mechanism is different. Someone who is not clinically allergic to a particular substance may develop symptoms simply from having too much of that substance in their system. A food sensitivity, for instance, is more likely to be 'dose dependent." For example, a given individual may be able to ingest one slice of wheat bread daily with no obvious symptoms, but the symptoms manifest after eating two slices in the same period.

I frequently encountered patients in my years of practice with the classic symptoms of fullness in the ear, tinnitus, fluctuating hearing loss, and either dizziness or true vertigo. The majority did not experience severe vertigo. For many, their symptoms largely abated when they removed all wheat products from their diets. Of the three symptoms, tinnitus was the least likely to go away completely. In my experience, wheat is the most problematic of all food sensitivities, especially in cultures where bread is a dietary staple. I also encountered more resistance from asking people to eliminate wheat from their diet than from any other recommendation. I also witnessed the most amazing results among those who followed this recommendation exactly. Note, I did not say to reduce one's wheat intake. I said to eliminate it from your diet.

Should you have these symptoms, I highly recommend that you immediately write down everything you've eaten during the preceding 48 to 72 hours. After three or four episodes, you'll discover a common pattern--some food or food additive that consistently occurs on each list. Then you know what to avoid in the future. Experiment to be certain. Totally eliminate that substance for at least three weeks, and see if your symptoms abate. Then add it back into your diet and observe what happens. Again, your symptoms may be quantity dependent. It may take several days of eating wheat again (if wheat is your offending substance) for your symptoms to reappear.

I also suggest that you look up all foodstuffs that contain wheat, corn, cow-dairy, and soy. In the U.S., nearly all packaged and processed foods contain wheat. MSG is a major ingredient of Soy Sauce, a common additive to Chinese food, and in most processed and fast foods. Aspartame is a common sweetener in diet sodas, packaged foods, medications, toothpaste, mouthwashes, etc.

With any of the symptoms detailed here, an examination by a qualified ENT specialist is essential to be certain of their cause. Once you understand the cause, however, prevention is safer and far more effective than any possible treatment. A significant part of your recovery from endolymphatic hydrops involves educating yourself about its possible causes. Also be aware that your doctor may not be aware of this information. If that's the case, he or she may resist the suggestion that "food allergy" is responsible for your symptoms. My suggestion to you is, try these suggestions anyway. You may be pleasantly surprised by the results.

If you notice that waxy buildup is covering the eardrum, however, you may wish to remove the wax yourself. Excessive earwax around the ear canal can put pressure on baby's eardrum, resulting in balance problems and severe discomfort.

Never insert a cotton swab, your finger, or any other cleaning implement into your child's ear canal. Even light contact is enough to scratch the eardrum. Moreover, cotton swabs can pack the dirt, dust, and other harmful particulate matter closer to the eardrum, actually increasing the risk of infection.

Some health care item manufacturers promote special bell-shaped swabs for cleaning the baby's outer ear. While these are safety-tested to present no danger to the child's inner ear, parents should still use them with caution.

Preventing waxy buildup

Earwax buildup can be slowed simply by keeping the child's ear and neck area clean. Parents can even use a cotton swab or ball dipped in warm water or peroxide around the ear bud and neck area for maximum cleanliness. Just make sure that none gets inside the ear canal.

When the child gets older

The child can begin cleaning its own ears as soon as it can understand the safe use of cotton swabs. Parents can introduce safe ear cleaning about the same time they teach the correct way to wash hair and trim fingernails and toenails.

One of the fastest ear infection cures is to add some supplementary nutrients into your diet. Vitamins and minerals can help boost your immunity, relieve your symptoms and stop an infection from recurring. I give all my patients with ear problems vitamins and minerals with good results.

Here's a summary of the nutrients that I use with my patients:


Ear Infection Cure 1: Vitamin C

I recommend my adult patients start on at least 3000 - 5000 mg of vitamin C a day to fight off infection. In young children (less than 2 years old) 1000 mg a day will suffice. For older children about 2000 mg works well.

Everyones requirements for vitamin C are unique so I suggest you start taking 500 mg of vitamin C every hour until your stools become a little watery or loose. When your stools become loose it means that your body is saturated with vitamin C. Any excess is excreted in the urine and stools. This point is called "Bowel tolerance" and is when vitamin C is helping your immune system most efficiently.

Going to just below your bowel tolerance every day should be adhered to until the infection is cleared from your body. Depending on the severity of the infection, this can take anywhere from a couple of days to a week or more. If your vitamin C intake is too low, you may not see improvements in your symptoms.

Ear Infection Cure 2: Zinc

Zinc stimulates your white blood cells and antibody production to fight off infections. This mineral is also a powerful anti-inflammatory that helps relieve the pain of sore ears, inflamed throat and swollen glands that are often present with ear problems.

For adults I suggest you start with 60 - 90 mg of elemental zinc a day. For young children I use 30 mg a day, for older kids 60 mg a day. If you also have a sore throat then dissolve a zinc lozenge in your mouth. A sore throat clears up faster when it has direct contact with zinc

Vitamin A and its precursor beta-carotene enhance immunity. Vitamin A has been shown to lower the rate of infections.

For immune support, I recommend 10,000 IU of vitamin A or 15 mg of beta-carotene a day. For young children about 2500 IU a day, and for older children 5000 IU a day of vitamin A is sufficient.

Vitamin A should not be taken in large amounts by pregnant women as higher amounts are linked to an increased risk of birth defects.

Other nutrients 

Optional but recommended nutrients I also dispense to my patients are vitamin E, selenium and the B complex.

Vitamin E helps immune function and is a powerful antioxidant. It is beneficial against both bacteria and viruses. I suggest at least 200 mg a day. If you have high blood pressure, check with your doctor first.

Ear Infection Cure 5: Selenium

Selenium is great to take alongside vitamin E, as they enhance each others absorption. It is a good immune booster. For adults take at 100 mg a day.

Ear Infection Cure 6: B complex

To help repair nerves in the ears a balanced B complex is advisable. The B vitamins also help you cope with stress and fatigue. A 25 - 50 mg B complex daily is a good start.

After the recommended use, the wax will eventually fall out or using a bulb or piston syringe with warm water to flush out that nasty wax.
